Fill a Photo / Model Release online — free and fully in your browser
A photo / model release is a short agreement where the person being photographed (the model or subject) gives a photographer or company permission to use their likeness — in a website, on social media, in advertising, in a portfolio, and so on. This template captures the essentials: who is releasing rights, to whom, for what uses, over what media, territory, and duration, and for what (if any) compensation. AttachKit fills it entirely in your browser — the PDF never leaves your device — and you sign it by hand or with the sign tool. It does not submit or file anything anywhere.
Your PDF never leaves your browser — open DevTools → Network and watch: the file is never uploaded.
Who needs it: Anyone who needs a subject's written permission before publishing photos or video of them: photographers, videographers, content creators, small businesses and marketers, event and portrait shooters, and the models or subjects themselves. If the subject is a minor, a parent or guardian signs on their behalf. For commercial advertising or other high-stakes use, have a lawyer review it — publicity and privacy rights vary by state.

General information, not legal or tax advice
This page is general information about a commonly-used document. State and local law varies — for advice on your specific situation, consult a licensed attorney in your jurisdiction. AttachKit fills the PDF; you're responsible for whether the contents are right for your case.
- Who signs a photo release — the photographer or the model?
- The model or subject (the person being photographed) signs, because they're the one granting permission to use their likeness. In this template the model/subject is the party whose details auto-fill. The photographer usually keeps the signed copy.
- What if the person being photographed is a minor?
- A parent or legal guardian signs on the minor's behalf. This template includes a separate parent/guardian signature line for that purpose. Rules for minors vary by state, so check your local requirements.
- Does a photo release have to be notarized?
- Generally no — a signed release is typically enough. Notarization isn't usually required, but requirements vary by state and by how the images will be used, so verify what applies to your situation.
- Is my information uploaded when I fill this out?
- No. AttachKit fills the PDF entirely in your browser and the file never leaves your device. It doesn't submit or file the release anywhere — you download the finished PDF and share it yourself.
