Fill and sign an employee warning notice online — free
An employee warning notice is the written record a supervisor uses to document a performance or conduct issue: the type of warning (verbal, written, or final), what happened, the policy involved, any prior warnings, the corrective action expected, and a deadline. It gives both sides a clear, dated account and a place for the employee to acknowledge receipt.

Who needs it: Managers, supervisors, and small-business owners who need to document a disciplinary conversation consistently. Useful for building a fair, dated paper trail before any further action, and for HR teams standardizing how warnings are recorded.

Employee warning notice questions, answered
- What's the difference between a verbal, written, and final warning?
- They're escalating steps many employers use: a verbal warning is a documented conversation, a written warning is a formal notice, and a final warning signals that further issues may lead to termination. There's no legally required sequence — pick the level that fits your handbook and the situation, and stay consistent across employees.
- Does the employee have to sign the warning?
- No. The employee's signature only confirms they received and discussed the notice, not that they agree with it. If they decline to sign, note the refusal and the date on the form and consider having a witness present. Employees can also attach a written response.
- Can I discipline or fire an at-will employee?
- In most states employment is at-will, meaning either side can end it at any time for any lawful reason — but you can't discipline for an illegal reason (such as discrimination or retaliation). At-will rules, final-pay timing, and required documentation vary by state and can be limited by a union contract or company policy, so verify your local rules.
- Is anything uploaded when I fill this out?
- No. AttachKit fills the PDF entirely in your browser — the file and everything you type stay on your device. It fills the form; it does not submit or file the notice anywhere. Download it and store or deliver it however your workplace handles records.
