Fill IRS Form 4506-T-EZ online — free

Form 4506-T-EZ is the short-form request for an individual tax return transcript — the simplest way to ask the IRS to mail you a free transcript of your Form 1040-series return. Mortgage lenders most often require it to verify the income on a home-loan application. It's simpler than the full Form 4506-T: it requests only the return transcript, only for individuals. Bring the blank IRS PDF (linked below); AttachKit auto-fills the common fields — the name shown on your return and your SSN — from your saved profile, on your device. You choose the years on line 6, then sign and mail or fax it. AttachKit does not submit the form to the IRS and does not give tax advice.

Your PDF never leaves your browser — open DevTools → Network and watch: the file is never uploaded.

Who needs it: Individuals a mortgage lender, school, or agency asks to verify a prior-year Form 1040 — most often home-loan applicants. If you need a business transcript, an account transcript, or a wage-and-income transcript, use the full Form 4506-T instead.

General information, not legal or tax advice

This page is general information about a commonly-used tax form. Tax law is complex and fact-specific — for advice on your return, consult a CPA, enrolled agent, or tax attorney. AttachKit fills the PDF; the IRS holds you responsible for what's on it.

4506-T-EZ vs 4506-T — which do I use?
Use 4506-T-EZ for a return transcript of your individual Form 1040. Use the full 4506-T if you need a different transcript type (account, record of account, wage and income, verification of non-filing) or a business return. AttachKit has both — see /fill/4506-t.
Is there a fee?
The IRS does not charge for a transcript requested on Form 4506-T-EZ. (A full copy of a return — Form 4506 — carries a fee.) AttachKit's fill tool is free; we add no IRS fee.
How long is my signed request valid?
The IRS notes the form must be received within 120 days of the date you sign it, or it may be rejected. Complete every applicable line before you sign — AttachKit leaves the year and transcript choices to you.
Does AttachKit send it to the IRS?
No. AttachKit fills and signs the form in your browser and gives you a PDF to download. You mail or fax it to the address in the form's instructions for your state; your PDF never uploads.
